Program Description:

This program provides direct clinical case management and therapeutic work with foster and adoptive children ages birth to 18. The work is conducted in the home on a weekly basis to address and alleviate mental health symptoms and behavioral problems that could lead to disruption of placements. It is also designed to ameliorate mental health problems that impair functioning and development and improve attachment relationships to their foster families or biological families, when appropriate. Crisis intervention is also provided as needed to the children. The goal is reunification and when that does not happen to provide permanency to the child. The ABW adoption Support Services Program offers assistance in all aspects of parenting prior to adoption as well as post adoption. A Better Way provides comprehensive clinical services to children and families, specifically those dealing with painful issues of physical, sexual and emotional abuse, neglect, abandonment and chronic intergenerational family trauma. Our Foster Family Agency provides treatment to children who have emotional and behavioral problems that can be remediated in a family setting. Trained Caseworkers and Therapists from Advent provide care and supervision to the foster children and foster parents. We attempt to include birth parents as an integral part of the treatment team, and look to reunite children with their birth parents. Should reunification be deemed inappropriate, Advent looks to establish permanent homes through adoption or long term foster care

Program Description: Advent's treatment services are designed to meet the needs of dually-diagnosed chemically dependent adolescents. Five group homes focused on 12-step recovery services and on-grounds schools also provide a drug-free environment in which to learn. A continuum of mental health and social services, emancipation training and family reunification services are also provided. Clients focus on recovery by attending daily group counseling, completing recovery assignments, going to 5 12-step meetings weekly, and working on relapse-prevention.

AFS Multidimensional Treatment Foster Care

Program Description:

AFS offers the Multidimensional Treatment Foster Care (MTFC) model developed at the University of Oregon to adolescents in Contra Costa County. Designed as an alternative intervention to residential treatment, Multidimensional Treatment Foster Care requires that a permanent home for the youth is identified prior to admission. The model utilizes a staff configuration that includes an individual therapist who works with the child, a skills trainer who works with the youth, a Parent Daily Reporter who works with the foster family and a Family Therapist who works to prepare the permanency family to accept the youth back into their home. Only one youth is placed per home in the MTFC program.

Program Description:

The goal of the AFS Extended Family Model of adoptions and therapeutic foster care is to provide children in need of family living situations with a diversity of opportunities to grow, trust, and bond within a family environment. We offer a rich network of supports to adoptive and foster parents including caseworkers, and tutors connecting every AFS child through the extended family to the community at large. AFS was founded in San Francisco in 1973 and is one of the first treatment foster care agencies in California and the United States. Our philosophy is that children belong in families preferably their family of origin, but if necessary to protect the childs safety, then in trained, stable, culturally competent and caring alternative families who will help the child recover from trauma and develop in a positive direction. AFS offers all services in both English and Spanish. Our Spanish-speaking services were developed with extensive input from Latino community members.

Program Description:

As the name suggests, Intensive Treatment Foster Care (ITFC) is foster care in which the services and supports to the foster homes are greatly intensified for children and youth who need extra structure and support. The intensified program might take the form of paid “support counselors” working within the foster home at specified time to provide additional support to foster parents and supervision to foster children. Or the intensified program might take the form of a more rigorous configuration of outside services that might include family reunification services, mentoring, targeted life skills training, or liaison with the schools. Generally only one child or youth is placed per home in ITFC though California law allows for certain exceptions to this rule.

The Girls and Boys Town Emergency Residential Childcare Center provides a needed safety net for children and families in crisis. The Girls and Boy Town Price Family Campus is a long-term residential program that provides therapeutic treatment services to girls ages 11-17. GBT offers a family style approach characterized by the presence of a married couple living in the home and serving as program managers. Youth length of stay averages approximately 1 year. Casa Pacificas 45 bed Childrens Crisis Care Center program provides a protective environment for Ventura and Santa Barbara Counties children who must be removed from their own homes or current foster placements pursuant to Section 300 of the Welfare and Institutions Code and who cannot be immediately returned home. The services provided consisit of an integraed and comprehensive array of services which meet the childs physical, emotional and development needs while being a resident of the facility. On-site Court school is rovided ot meet the needs of our children. Complemented by county staff from various departments, the Childrens Crisis Care center program also funcions as an assessment and diagnostic center for these children

Program Description:

The Non-Public School serves eligible students, ages 10-18, from our Childrens Crisis Center, Residental Treatment Center, as well as, Day School students referred by school districts. In order for students to be eligible for the Non-Public Scchool, they must have an Individual Education Plan qualifying them as Emotionally Disturbed, Severly Learning Handicapped or Other Health Impared (such as ADHD). In addition, they must have been found elgible for a non-public school due to their inability to funciton in a less restrictive setting (i.e. off-grounds public school). Roughly 70% of our students live in the RTC, 5% in the CCC, and 25% at home.The school program offers a full day of academic course work, and incorporates goals established at a students annual Individual Education Plan (I.E.P.) meeting. The school hours are 8:30 2:30. All of our classrooms are self-contained with students floating to different classes only for electives. Each classroom curriculum uses as its base the courses offered in the local school districts curriculums frameworks and guidelines.All teachers are appropriatley credentialed and work with an experienced teaching asistand and behavioral specialist. There is a maximum of 9 students in each classroom. The behavioral staff serve as a part of the team in each classroom. They offer a continuum of services ranging from assisting with tutoring, offering reinforcement and problem solving when difficulties occur, to intense crisis intervention. Tutors are also available for futher acaedmeic assistance.We also offer a wide range of opportunities for enrichment. Our music program is taught by trained musicians and offers music training as well as performance opportunities. Youth must meet medical necessity for a mental health disorder. THERAPEUTIC BEHAVIORAL SERVICES (TBS) are available to children already receiving services through the Mental Health Program who are at risk of losing their current placement and going to a higher level of care. TBS provides one-to-one coaching in behavioral skills and rehabilitation counseling to reduce target behaviors. Services are brief, structured to address a specific behavior. Interventions are designed to reduce the specifically measured target behaviors and to increase alternative skills and preferred optional behaviors. Frequency, Intensity and Duration of targeted behaviors are measured weekly.

Program Description:

The Comprehensive Assessment and Stabilization Service (CASS) program serves foster children in foster homes, small family homes and small group homes who are at risk for placement disruption. Staffed by a team that includes psychiatrists, licensed psychologists, therapists and paraprofessional behavior specialists, the CASS program provides crisis intervention, consultation, and mental-health and case-management services, as well as medication evaluation and monitoring in order to stabilize the foster youth’s current placement. Services may be provided to youth and caretakers. Services are generally home- or community-based, and are well-integrated with regional department of social service case workers. Short-term in nature, services are designed to address immediate needs impacting continued foster placement with linkages created for longer term support of the foster youth. Coolidge Court provides 18 units of assisted independent living for qualified tenants. Tenants are typically between 18 and 21 at the initiation of tenancy, have been recently in the foster care system and typically have an axis one mental health diagnosis that impedes their smooth transition into adult life. Program Description:

Premised on our understanding that families frequently experience confirmation of their fear that they lack what is needed to meet the needs of their child when crisis erupts, we designed this program explicitly as an alternative to psychiatric hospitalization. In this program a family therapist works with a family for six to eight weeks for multiple hours weekly as needed to alleviate and develop effective responses to crisis. Entering our seventh year of operation, we serve approximately 90 families annually in San Mateo County. Replicating our successful In-Home program in San Mateo County, we have two years experience providing In-Home Stabilization and Therapy services to young people in Alameda County who are involved with Child Welfare Services or Juvenile Justice. This program draws on the evidence-based practice of Functional Family Therapy to provide strength-based support to families, foster families and other caregivers of young people at crisis points. Crisis may stem from an intensification of symptoms, a developmental change, or a change in the family’s circumstance. Therapists work with young people and their caregivers to foster resiliency and develop and implement strategies to regain stable functioning. Program Description:

FFYC provides comprehensive residential treatment for youth who dually diagnosed with developmental disabilities and mental health challenges. With three diverse locations, our program houses 40 young people, providing safety, education, and behavioral and mental-health services. The program emphasizes building on youth strengths toward successful community re-entry, including vocational and independent-living strategies as well as comprehensive family therapy services. Services include day treatment, mental health services, behavior analysis and intervention, full-range special education services, group, individual and family therapy and community living skills. Located in Vacaville, Oakland, and San Diego, program sites provide youth with treatment while keeping them as close as possible to the community of their family This program is accomplished through careful collaboration with public agencies and ourselves. Working in each county with the local school district and SELPA, with the local regional center, child welfare, juvenile justice, and mental health departments, this program provides services to young people and families who have not fit into traditional care and treatment models. Defying expectations that cognitively impaired young people cannot benefit from therapy, this program uses a combination of behavioral intervention, play therapy and intensive family therapy to enable young people to maximize their capacity for self-control, develop and realize realistic life plans that allow for stability in direction and living situation and permanence in connection with caring others, and develop age-appropriate relationships with parents and siblings. Program Description:

Therapeutic Behavior Services (TBS) provide one-on-one behavioral intervention to qualifying children and youth to support their success in current placements, prevent moves to higher levels of care and to increase stabilization as youth return home or to lower levels of care following successful placement. Designed as an adjunctive therapy to address short-term behavior change essential to proceed with on-going treatment, this program works closely with young people to understand behavior and accomplish change and with caregivers to assure ability to maintain systems that support the young person’s change. Currently FFYC provides TBS to more than 60 young people per day with contracts from six California counties. The Transition Age Youth (TAY) Partnership is a collaborative initiative that joins the resources of FFYC with those of two other agencies, the Greater Richmond Interfaith Program (GRIP) and The Latina Center, as well as Contra Costa County Mental Health Services, in a new program (awarded December 2006) under the Mental Health Services Act. With FFYC acting as the lead agency, the TAY Partnership serves 135 concurrently engaged transition-age youth in West Contra Costa County, focusing on unserved or underserved TAY, especially homeless youth. The TAY Partnership accompanies and assists TAY through their recovery journeys, employing a “whatever-it-takes” commitment to positive outcomes. We provide a continuum of housing (administered by the County) and services, including shelter care, transitional housing, permanent housing, satellite housing support, access to substance-abuse treatment in-patient resources, and family support. Other approaches to develop resilient, socially minded self-advocates include vocational, educational, health, and psychiatric services, financial mentoring, advocacy, transportation, and rehabilitative visits from staff. FFYC’s VTA program provides individual and collateral therapy to children and youth in the community. Inland Empire Residential Centers operates two 14 bed homes in Redlands, California. We offer a highly structured residential treatment program for boys who have emotional, behavior, and/or education concerns. Social work services are provided by two full-time social workers on-site for each home supervised by an LCSW. We welcome adoptive families seeking support for their children who may need structure and discipline that promote a healthy permanent reunification. Recognized by counties and courts for our ability to proactively carryout programs that transform lives, the Residential Treatment Program employs innovative intervention techniques that focus on progress toward individual treatment goals. Psychiatric, medical, psychological, rehabilitative and nursing services are part of our comprehensive program. Six cottages housing 12 boys each provide family-like surroundings to 72 children as they make their sojourn within the child welfare system. Each cottage is equipped with a kitchen, dining area and den, all of which typify the welcoming comforts of home. Our multi-disciplinary, professional staff is available to work with our children 24-hours a day. Our 21-acre campus is nestled against the backdrop of the San Gabriel Mountains at the east end of Los Angeles County. Living in a nurturing cottage environment children experience a sense of belonging where the emphasis is placed on personal responsibility. Children learn skills enabling them to build and maintain healthy relationships with peers and adults by developing rational and effective ways of dealing with painful, stressful situations. An overarching goal for all of the children in our care is positive engagement with adults and peers.

Barack Obama Academy is a middle school program run by the Oakland Unified School district at which Lincoln provides direct mental health services to the students and assists the teachers and administrators. The program implements a three-tiered approach. The first tier includes school-wide services designed to prevent maladaptive behavior and reinforce positive behavior throughout the school community. The second tier includes classroom-based services designed to prevent and/or ameliorate problems specific to each classroom. Short of hospitalization or a locked facility, Lincoln’s programs are designed to offer the highest level of intensive mental health treatment available for children who cannot live at home. The primary goal of Lincoln’s Residential Treatment Program is the successful reintegration of children into the family of origin or into an alternative living setting. Reunification with a family should not pose any significant developmental interference for the child, and should support the child’s progress towards maturation and independence. In lieu of returning home, the child is aided in to securing long-term placement stability. Milhous Children’s Services provides care and treatment for youth who have psychological, emotional, or behavioral disorders. Our programs and facilities are designed to divert children and adolescents that are in crisis from restrictive environments by providing mental health intervention and crisis resolutions in a safe and secure setting. Our specialized programs offer children a structured environment with consistent supervision. The contracted consulting psychiatrists work in conjunction with our clinical residential social workers and other mental health staff who design, implements, and monitor the treatment plan for each resident. The mental health services are scheduled to treat the residents 24 hours a day - 365 days of the year. The primary goal is to provide children in crisis with immediate intervention, while providing an initial evaluation for their immediate emotional needs. Oak Grove offers a 24-hours therapeutic program for boys and girls requiring long term care. Partial Hospitalization Oak Grove also offers a Monday through Friday program for individuals who do not require daily 24 hour care. Successful boys generally stay for 18 to 48 months, with many leaving as 18 year old High School graduates.

The program at Pacific Lodge provides a safe, predictable, organized and supportive environment where residents are taught and encouraged to overcome maladaptive and self defeating behaviors and are challenged to acquire and practice pro-social behaviors. Pacific Lodge’s highly structured residential treatment program requires the boys to participate in personalized program components. The clinical services provided by in-house professional staff are the core elements of treatment for each resident at Pacific Lodge. A carefully selected group of professionals with masters or doctoral degrees provides individual, group and family therapy. The aim of Pacific Lodge is to fully understand each child’s psychosocial make-up. For this reason each resident admitted to Pacific Lodge receives the following assessment: • Psychiatric evaluation • Psychosocial evaluation • Educational assessment • Vocational assessment • Alcohol and chemical dependency assessment • Nutritional assessment • Complete physical and medical history evaluation Our consulting psychiatrist assesses boys at Pacific Lodge within the first week of admission. A multidisciplinary treatment team regularly assesses each boy every 90 days in conjunction with the required Needs and Services plan, inviting all stakeholders to participate in the formation of a comprehensive treatment plan. In addition, the cottage supervisor, case manager and resident meet with school personnel every 6-8 weeks for joint educational assessment. Recognizing that each resident is unique, Pacific Lodge customizes an individual treatment plan for each youth. These treatment elements help residents develop communication and relationship skills, strengthen personal identity and foster pro social attitudes and behaviors. Based on the residents’ ages and needs, they are placed in one of the four residential cottages. Each cottage is staffed with two therapists, a case manager, and eight child care specialist who provide the therapeutic care and treatment for each boy and monitor and coordinate the ancillary services prescribed in the treatment plan. Evidence based measures are maintained by various members of the treatment team and reflect individual growth in the areas of psycho-social functioning, compliance with rules, emotional development in therapy, educational development in reading, math skills and classroom expectations. Each resident’s progress is measured by his participation in individual and peer group sessions, integration into the cottage milieu, and performance and behavior within the elements of his personal program. The treatment team reviews clinical reports monthly and as the individual needs of the child change and/or evolve, re-assessments are conducted to make sure the treatment program remains appropriate for the boy. Residents’ achievement in sports, activities, work programs and community educational programs are also factors used in measuring outcomes. On going written reports from families following home visits provide data for evaluation of developing family communication and cohesiveness and are discussed as part of family therapy. All measures and assessments are provided to the placing worker in the monthly visits, as well as in Quarterly report formats, which discuss the above assessments and outcome/results Pacific Lodge – A catalyst for change The day to day structure within our treatment program relies on Behavior Modification to initiate change in negative behaviors. There are clearly defined expectations throughout the program which emphasize pro-social norms. For example: with positive behavior boys are able to earn additional privileges in their program. If a boy engages in negative behavior they will receive restriction of privileges or work-related consequences. Through this structure, our boys are provided with consistent and predictable limits which create a safe environment for all. Our mission is to: Acknowledge the belief that every facet of a boy’s day provides him with the opportunity for meaningful learning and change. Provide our residents with the highest quality professional, licensed, and supportive treatment program in the most cost-effective manner possible. Establish treatment goals to include individual, group, and family therapy, psychological testing and psychiatric consultation. Provide an accredited and individualized academic program. Help each child realize his full potential by offering vocational training, social skills development, and exposure to a wide variety of well-supervised activities that will lead to the development of tangible skills, build self-esteem, and a bright future. Provide support necessary to assist each boy to become self sufficient, reunited with family or transitioned to independent living. Our transitional housing program provides former foster youth a smooth transition from direct care to independent living. Crossroads is a Level 10 community integrated group living program for girls ranging in age 7-17 years old. The girls and young women in this program have been unable to live in a natural or foster home setting. They are referred through social services, mental health and in rare cases, probation. Redwood Children’s Services Inc. is a non-profit public benefit organization established to create placement, counseling, and community service options for foster children and their families. The RCS Foster Family Agency (FFA) provides living arrangements and support services to children, primarily from Mendocino, Lake, and Sonoma Counties, and to foster families who have passed our ACT (Advanced Certification Training) certification requirements. Our desire is to provide community based therapeutic foster homes to children and youth as the least restrictive alternative to out of home placement. The Sacramento Crisis Nurseries offer shelter care for children and support to families experiencing a crisis in two Sacramento County locations. Children (birth through age 5) can stay up to 30 days while their parents seek help and long term solutions to their problems. The majority of parents coming to the Sacramento Crisis Nursery North (SCNN) are young, single mothers on welfare who are often impacted by medical issues, mental illness, post partum depression and isolation. Mothers bring their children to the nursery because they have few friends, relatives, or other resources to help when they are in crisis. Hospital social workers place children at the crisis nursery when a mom must be hospitalized and there is no one to care for their children. Moms come to the nursery when they have a mental health diagnosis and must be stabilized on medication before they can parent their child. Women with newborns call the nursery with post partum depression and can only seek help if they have a safe place to leave their infant. Parents can access emergency day respite from 7:00 a.m. to 7:00 p.m. or 24 hour respite for up to 30 days. The Case Manager provides counseling, support and referrals to needed services. The Case Manager works with hospital social workers, mental health providers, and support groups to aid these young women who have unmet health needs. These women will not be able to successfully parent their infant or young child without resolving the health and mental health issues they are experiencing. The Community Treatment Facility (CTF) provides 24-hour care and treatment for the most severely disturbed adolescents in San Francisco County. The CTF was created in order to provide these youth and their families with a local, community-based alternative to psychiatric hospitalization or placement in an out-of-state secure facility. The CTF combines strong behavioral and learning interventions with a treatment approach that enhances and builds upon peer, family, and staff relationships, promoting a nurturing and safe environment where young people and their families can heal and move forward.

Program Description:

Family Finding is the practice of providing a child with the life-long connections that only a family can offer, through seeking out relatives who the child is not currently connected with. Through locating, approaching and partnering with these relatives, children may gain the family support and sense of belonging that every child needs. Seneca Center provides family finding services for children placed or enrolled in many of its programs. The agency has also partnered with Kevin Campbell, who incepted and developed the practice of Family Finding, to create the Center for Family Finding and Youth Connectedness. The goals for the Center include: (1) to connect our most isolated youth to family members who can love and support them; (2) to design and implement technology to track Family Finding progress and outcomes; and (3) to advocate for changes in larger systems of care that will promote family unity and lifelong connections for children. Currently, Kevin Campbell and the Center provide Family Finding trainings for social service and child welfare agencies located throughout the United States and British Columbia.

Family Finding Evaluation (Research Project)

Program Description:

In partnership with the research organization Child Trends and multiple partner agencies throughout the country, Seneca Center is participating in a rigorous experimentally designed evaluation of Family Finding to establish best practices, inform program design and examine Family Finding’s impacts on the lives of children and youth. The evaluation will be conducted across 12 programs using random assignment and control and involving a minimum of 300 foster children (150 treatment group, 150 control group). The design will have two major approaches for impact measurement as well as a process study. The first source of impact information will be child welfare administrative data. This data will be collected for all children and youth in the evaluation in order to examine changes in children’s permanent placements and placement restrictiveness, allowing for a comparison across treatment and control groups. The second source of information will be in-person interviews with youth to examine the impact of Family Finding on children’s well being, including education, health, mental health and psychosocial wellbeing. The process component will consist of site visits to each of the participating program sites, interviews with administrators and managers, focus groups with referring caseworkers and Family Finding workers and interviews with community stakeholders.

The ITFC program, working to provide services throughout the Bay Area, offers a family-based alternative to group care, utilizing trained and highly-supported foster parents as the primary caregivers. Program support includes intensive staffing: social workers provide case management and therapy for children and families, as well as supervising support counselors who work one-on-one with children in their foster homes, schools, and communities. Seneca staff psychologists, psychiatrists, and education specialists provide consultation to ITFC program staff, as needed, to address the needs of foster youth and their caregivers.

Program Description:

Mobile Response Teams (MRT) support families throughout Alameda, Contra Costa Counties in serious distress with immediate crisis intervention and mental health services. The teams travel to where the youth and family are in the community, in order to provide services at the time of the crisis. This can occur in family homes, schools, hospitals, or other community and residential settings. Therapeutic interventions by the teams are primarily centered on assessing the immediate safety needs of the family, stabilizing the youth in crisis, and providing assistance and support to the caregivers. Follow-up linkage services are provided in order to ensure youth and family connections with longer-term mental health services, if needed.

Seneca collaborates with the Alameda County and San Francisco County Probation Departments and Behavioral Health Care Services to provide Multisystemic Therapy (MST) to youth and their families. MST is an evidence-based practice that utilizes intensive, strengths-based therapy provided in the home or community of each client youth and family. The average treatment duration is five to seven months. The focus of the program is to build upon family and community strengths to address difficulties the youth is experiencing in school, with peers, and in family relationships. Addressing these key areas reduces the likelihood of re-involvement with the juvenile justice system.

Program Description:

Willow Rock Center is an adolescent crisis stabilization and treatment facility provided in partnership by Seneca, Telecare Corporation, and Alameda County Behavioral Health Care Services. Providing services within Alameda County, Willow Rock has three components: a 23-hour crisis assessment center (Seneca), an acute inpatient program (Telecare), and an outpatient program (Seneca) designed to transition adolescents back into the community with links to needed supports. Seneca’s 23-hour assessment center is the front-end of the crisis stabilization continuum of care, and is designed to provide the immediate intervention services necessary to divert adolescents from hospitalization. Program staff provide a safe and nurturing environment as they assist the youth in regaining the stability necessary to return home or to a community-based placement. Seneca’s Intensive Outpatient program operates as the back-end service to the crisis stabilization continuum of care. Intensive outpatient services are available to youth coming from either an inpatient setting or the 23-hour assessment center. Intensive outpatient services may last from several weeks to 12 months depending on the needs of the youth.

FOCUS (Family Outreach Community Support) is defined as an intensive mental health program serving children, youth and families in Sacramento County with a high level need for mental health treatment and community support. A wide array of mental health services and supports that employ Stanford Home Family Centered Practice principles are utilized. The family-centered approach is an individualized, strength-based, outcome driven, and sometimes unconventional response to complex family needs. Intensive mental health services and psychotherapeutic interventions are delivered in homes, schools, and community settings by a FOCUS treatment team consisting of a licensed or license eligible clinician, a child and family team facilitator (mental health rehabilitation specialist), and a family advocate. FOCUS program outcomes include decreasing Minor Emergency Response Team (MERT) contacts, psychiatric hospitalizations, and out-of-home placements; while strengthening family connections, improving mental health functioning, and connecting family members to community based resources and supports.

Stanford Home for Children’s foster care program is designed to achieve permanency for children and youth who require either temporary or permanent out-of-home placement. Every child served should either reunify with birth family or be placed with a family that will make a lifetime commitment to the child, preferably through adoption. The program contributes to permanency through Standard Treatment Foster Care and Intensive Treatment Foster Care. Children have a better chance of achieving permanency when they live in families rather than group care facilities. Stanford Home provides Intensive Treatment Foster Care, designed to stabilize children with challenging behaviors in a family based setting. Placement in ITFC homes can be used as a resource to prevent placement in a group care facility as well as to move children out of group care facilities.

TBS provides children/youth with community based intensive one-on-one short-term therapeutic contact for a specified period of time in order to maintain the child/youth’s placement at the lowest possible level by resolving specific target behaviors and achieving short-term treatment goals. This program is one component in Stanford Home’s commitment to prevent unnecessary residential placements, psychiatric hospitalizations, and discharges to higher levels of care.

Program Description:

Wraparound is a program that combines a (1) family-centered, strengths-based, needs-driven, team-driven approach that literally “wraps” services around a family with (2) individualized mental health treatment for children. The Wraparound approach is a comprehensive, individualized, outcome driven, flexibly funded, and sometimes unconventional response to complex family needs. Mental health treatment refers to individualized diagnosis, planning, and intervention to improve each client’s behavior and functioning. Desired Outcomes In Wraparound, desired outcomes are determined on an individualized basis, family by family, team by team. Within the system of care, Wraparound may be said to have the following desired outcomes: 1. To have families achieve their goals, usually with their children living in the family home or some other permanent family setting. 2. To function as a safe, community-based, family centered alternative to high level Residential Based Service placements (RCL 10-14). a. To decrease children’s length of stay in Residential Based Services (RBS) settings. b. To avoid first time or subsequent placements of children in RBS care. c. To move children into family settings as soon as it is safely and legally possible. 3. To place children in communities where their families reside, if not with their family. 4. To provide sustainable, permanent family connections and living arrangements regardless of age. 5. To achieve an overall adjustment and improvement in the youth’s behavior and functioning, through a demonstrated use of strength based adaptive behaviors and an increase in the use of functional social skills. 6. To have families utilize less wraparound funded, targeted, categorical services and more family-based, community-based, universally available resources over time in the Wraparound program. 7. To have families in the wraparound program utilize resources at costs less than the cost of placing the children in high level Residential Based Service placements (RCL 10-14). Principles and values Wraparound is committed to offering a program consistent with the following principles and values. 1. The family unit is our focus of attention. Family-centered practice means that we work with the family as the primary focus of attention, support and intervention, promoting the safety, well-being and permanent connections between family members, including children identified as needing specific care and attention. Caregivers and siblings receive services in accordance with the needs of the family and the wishes of a family-driven team. Unbiased information is shared with families on a continual basis in order to support their need and ability to make decisions with adequate information. True family/professional collaboration is sought, not just coordination, recognizing that the family should always be the constant in a child’s life. 2. We strengthen the existing capacities of families to function effectively. The primary purpose of family-centered practice is to strengthen the family's potential for carrying out their responsibilities by focusing on family strengths, not on problems or deficits. These strengths are also leveraged to address challenges and needs identified by the family. 3. Families are our full partners in designing all aspects of policies, services, and program evaluation. Summitview is a nonprofit organization licensed by the state of California to provide mental health services to children and their families. These services include level 14 residential treatment, Medi-cal Day Rehabilitation, as well as outpatient therapy, TBS and Wraparound services for boys and girls. We also provide nonpublic school services. Summitview provides residential treatment for girls from the ages of 10-17. The primary therapeutic approach used in the residential program is Dialetical Behavior Therapy (DBT). DBT is a state of the art cognitive behavior therapy that has been shown in clinical trials to be effective in the treatment of chronically suicidal females. The goals of treatment are to decrease life-threatening behavior, reduce impulsiveness, reduce emotional reactivity, and improve social functioning. Summitview School is a certified nonpublic school that provides individualized special education services. The school provides a full curriculum of academics taught by fully credentialed special education teachers. Last, Summitview also offers outpatient counseling and other speciality mental health services to families living at home or in foster homes who may be struggling with a variety of interpersonal or behavioral issues.

Coast is an 8 bed all-female residential program, housed in a residential neighborhood one mile from a non-public school. There is an overnight staff and 2-3 counselors on duty during awake hours. Residents of the house generally attend the NPS that has 40 students. Residents of Coast have been abandoned, abused, and/or have significant emotional problems. Some also have learning disabilities. The program combines a structured behavioral system with an interpersonal approach to treatment. The average stay is 12-18 months. Coast House residents share a bedroom with one other teen. Residents share responsibility for chores such as dinner preparation and house cleaning. All residents have an emancipation plan, which includes schooling and a part-time job. While the program is a level 12 and uses an awake-night staff, residents are expected to have good behavioral controls and to be motivated. An on-grounds NPS is available as is public school, adult school, and a local junior college

Transitions Children's Services (TCS) was founded in the summer of 2009 by Brian Van Anne, MSW and Joe T. Martinez, CPA. Mr. Van Anne brings close to fifteen years of social work experience to this agency, eight of which he served as a foster family agency social worker, foster family agency supervisor and foster family agency director for another local, well respected foster family agency. Mr. Van Anne has a Bachelor of Arts degree in Social Work and Master of Social Work degree from California State University, Fresno. Mr. Martinez brings over twenty years worth of experience in financial management and oversight of companies ranging from large for-profit entities to social service, county funded programs. Mr. Martinez has a Bachelor of Science degree from California State University, Fresno and a Certified Public Accounting Certificate from the California State Board of Accountancy. Mr. Martinez also has tremendous love for children and feels that this agency will serve and an excellent way to improve the lives of those who have been abused, neglected, or abandoned. TCS’ purpose is to provide quality, culturally competent treatment foster care services to children and families in the child welfare/foster care system. TCS will partner in philosophy and delivery of service with surrounding counties in order to meet the needs of their consumers. TCS aims to use a Strength Based approach in recruiting, training and delivering foster care services with the underlying theme being that foster care is a temporary living arrangement for children; reunification is the goal and cornerstone of case planning which will be supported throughout the treatment plan process. TCS will offer case management services in the form of weekly home visits, the formulation of treatment plans, foster parent support services and crisis intervention services. TCS social workers will be expected to make at least bi-weekly home visits to the foster home making face to face contact with the consumer in placement. In most cases, TCS social workers will make weekly contact to ensure the highest quality of oversight possible. After each contact, TCS social workers will write a narrative in the form of a case note documenting the nature of the visit. Treatment plans such as Needs & Service Plans and Quarterly Reports are done at intake and every three months there after. These assessments will serve as the foundation of the treatment plan process. Also, TCS social workers will be responsible for writing Incident Reports, when any unusual or unfortunate incident occurs and Transition Reports, when a consumer is discharged from the agency. All reports will be formulated by TCS social workers with input from consumers (based on their age and ability to formulate cognitive thoughts) county case managers, foster parents, mental health clinicians, school personal, biological family members, court officials and any other authorized representatives. During, but not limited to home visits, TCS social workers will offer support to foster parents. This might be in the form of face to face conversation about the consumer or target goal the team is striving to achieve, assistance to new foster families who might have questions about the foster care process, reassurance and support to a foster parent who might be questioning his/her ability to foster or could be in the form of transportation or supervision of a consumer. TCS social workers will be available around the clock to provide crisis intervention services. This includes the assigned social worker during normal business hours and a rotating emergency response cell phone allowing a TCS representative to be available to any county personnel, foster child or foster family twenty-four hours a day. TCS social workers will be Masters level graduates in compliance with CCR 88065.3(b) (1). This level of education will allow for social workers to provide therapeutic intervention during normal and crises situations. TCS social workers will assist in the formulation of treatment goals that will be addressed by the treatment team (to consist of TCS social worker, county case manager, consumer, foster parent and any other authorized representative). TCS will also partner with the County and incorporate their intended services for the consumer like mental health counseling, psychiatric assessment and services, biological family visits and treatment team staffings. TCS will recruit, train and certify perspective foster families under our State issued facility number. We will use a comprehensive training model that addresses CCR regulations (88088(5)(B) and 88069.7(c)(5), including but not limited to initial and ongoing training in child abuse identification, prevention and treatment. TCS will also incorporate training on Fresno County’s Family to Family initiative; the core being that foster families serve as role models and offer support to families during the reunification process.

Triad has approximately 200 foster homes in Northern California that serve more than 260 children; in a single year, 1400 children call Triad “home” for at least one night. Twelve years ago, the average age of a Triad child was 16 1/2 years; today the average age is just under eight years. Triad is often asked to accommodate special-need situations: providing help for families with children as young as one day old or providing care for young single mothers with infants. FFA not only benefits the children already a part of Unity Care, but many other children who have not had the opportunity to blossom in an atmosphere of love, warmth, and understanding. With a treatment team of professional, experienced social workers dedicated to changing the guture of a child, Unity Care has developed a formula for success. Professional services are designed to meet the needs and social development of our children. We provide these services through trained foster parents in our Certified Foster Family Homes. In 1991 Valley Teen Ranch was licensed as a Foster Family Agency and now certifies over 25 homes in Fresno, Madera, Tulare and Kings Couties in the Central San Joaquin Valley. Our foster families have nurtured more than 400 children and on the average we have more than 20 children in our care at any given time. Out of these children in the Foster Family Agency, Valley Teen Ranch arranges several adoptions per year thru our adoption agency. Happy endings are what Valley Teen Ranch is all about. Valley Teen Ranch provides recruitment, training, certification and placement for boys, girls and sibling groups in our Foster Family Agency. There is a ratio of one Masters Degree Social Worker for every 15 children in care. There are weekly face to face visits to our foster homes, case planning and case management for these foster children. We are the liaisonn to the biological family, the school, the court and the Medical Doctors and Dentists, too.

In 2002 Valley Teen Ranch opened a Transitional Living Home to house four young men from various local youth programs, age 18-21 who have no place to go and would otherwise be homeless. To live in the house for two years, the young men must undergo random drug testing, be in school and have a job. They are treated as adults to make many of their life decisions, but they must abide by basic rules and are monitored by a mentor who stays in contact during the day and who sleeps in the home at night. The young men participate in education, employment, individual counseling, independent living skills, drug and alcohol treatment and social skills training.
